What is end of life care like in South Yorkshire?

We want to find out about experiences of end of life care in South Yorkshire. 

The project has been commissioned by NHS South Yorkshire, to help them plan and improve palliative and end of life care services in our area, working with the partners including hospitals, hospices, and community services.

We want to make sure people's views and experiences are included in this work, so we are asking what is working well and what would make experiences better for people. We will be sharing people's anonymous feedback with NHS decision makers, and it will inform their plans to make improvements in Sheffield and the rest of South Yorkshire.

How can you share your views?

We are running an online survey to find out more about people's experiences. You can complete the survey if you :

  • are currently using palliative or end-of-life care services
  • know someone who is
  • or if you have been bereaved in the last 2 years

End of life care survey

The survey should take about 10 to 20 minutes to complete. It's anonymous, which means you don't have to share any personal details about yourself unless you want to.
